Accurate nutrition assessment is essential to optimizing growth, development, and clinical outcomes in neonates, particularly in preterm and very low birth weight infants. In the neonatal intensive care unit (NICU), nutritional status is highly dynamic and influenced by complex interactions among medical conditions, developmental physiology, and nutrient delivery. A structured, evidence-based approach enables early identification of nutritional risk and supports targeted, individualized care.
Neonatal nutrition assessment is commonly organized using the A–B–C–D–E framework, which integrates anthropometric, biochemical, clinical, and dietary intake data, followed by evaluation of findings. This multidimensional approach ensures that clinical decisions are informed by the integration of multiple data sources rather than reliance on a single measurement.
Growth remains the primary indicator of nutritional adequacy, with serial measurements of weight, length, and head circumference providing insight into fluid balance, lean tissue accretion, and brain development. Biochemical and clinical assessments add critical context, helping to identify metabolic abnormalities and functional status while reinforcing the importance of interpreting trends over time rather than isolated values.
Dietary intake assessment requires careful evaluation of all nutrient sources, including parenteral nutrition, enteral feedings, intravenous fluids, and supplements, with intake interpreted relative to the infant’s needs and clinical condition. Integration of these components supports identification of neonatal malnutrition using indicators such as nutrient intake, growth velocity, and changes in Z-scores.
Together, this structured approach supports informed clinical decision-making and enables nutrition strategies that promote optimal growth and long-term developmental outcomes.

Michelle Johnson is a Registered Dietitian Nutritionist and Research Scientist at Abbott, where she provides global clinical and scientific leadership for Abbott’s infant nutrition products. At Abbott, Michelle participates in pediatric clinical research and product development while providing scientific education for Abbott’s infant nutrition products. Prior to joining Abbott in 2014, Michelle was a neonatal and pediatric nutritionist for 17 years in multiple neonatal intensive care units and pediatric hospitals in the U.S. Her areas of expertise include neonatal and pediatric clinical nutrition. Michelle is an editor for the Pocket Guide to Neonatal Nutrition, 3rd edition, author for The Academy of Nutrition and Dietetics on the topic of neonatal nutrition and is an author of peer-reviewed scientific publications and consumer articles on infant and preterm infant nutrition. She is passionate about nutrition care for the preterm infant and the importance of nutrition for improved outcomes.
